Thank you for requesting a boarding appointment with Memorial Beach Veterinary Hospital. We look forward to meeting all of your pet lodging and veterinary needs.

Please note: You may be required to sign additional release forms in our office, as well as provide us with a copy of your pet's vaccination certificates.

Boarding Policies:

• Vaccination Requirements
  For your pet’s protection, all vaccines must be current. We require written proof or phone confirmation from your veterinarian of vaccinations. Dogs must be vaccinated for Rabies, DAPP, and Bordetella. Bordetella for dogs must be current within 6 months. Cats must be vaccinated for Rabies, Leukemia, FVRCP and bordetella. Please note that some vaccines may not take affect for several days, so be sure your pet is vaccinated before boarding for optimal wellness. Your pet also must be free of internal and external parasites. If parasites are found while boarding, they will be treated at the owners expense.
• Additional Policies
  I understand that if any of the following conditions are found they will be treated at the owner’s expense:
  • External parasites: including fleas & ticks
  • Intestinal parasites: including tapeworms
  • Any condition causing your pet distress or discomfort
  • Any emergency medical condition.

  I authorize MBVH to bring my pet up to date on any required vaccines if they are not current. (proof of vaccines required)

• Boarding Charges:
  Owners will be charged a full day boarding the day the pet is dropped off. There are no charges for the last day if the pet is picked up before 1 p.m. Pick ups and Drop off hours are 9-1 p.m. M-Th & Sat. Friday 9:30 to 1 p.m. No pick ups Sundays and most Holidays. 
• Pet Belongings:
  Memorial Beach Veterinary Hospital can not guarantee the return of pet’s belongings (toys, bedding, etc) in the same condition as when they came in. Memorial Beach Veterinary Hospital is not responsible for any losses or damages to belongings you choose to leave. In order to entertain your pet and keep them comfortable, we will provide toys and bedding for them during their stay.
• Check Out: Check out time before charges are accrued for the next business day is 1 p.m.
                   No drop offs or  pick ups on Sunday or holidays

Please remember that your appointment is not final until you receive confirmation from our staff.

Minimum Stay:  2 night minimum

Deposit: 2 night deposit with reservation. Refundable up to 72 hours before arrival.
